By purchasing a plan through Haler Health you agree to the standard terms and conditions (below) and the Extra Condition (dental practice specific conditions) which are stated in the plan description.

Standard Terms and conditions

Accessing benefits
You are eligible to receive all benefits immediately upon sign-up. Simply, contact the dental practice and tell them you are Haler Health plan with them and want to schedule an appointment. You can share plan benefits with members of your direct ​family,

The discounts contained herein may not be used in conjunction with any other discount plan or program.

Plan term is for one year. After a year has passed, and if the plan is still available, the benefits reset. Plan benefits expire after 1 year (no rollovers).

​Your plan is dentist practice specific. Scale and polish treatments may be done by dentist or hygienist depending on availability.

If you contact your dental practice and you cannot get an appointment within 30 days (excluding 15 Dec to 15 Jan) - you have grounds to cancel your plan. 

Cancellations & Refunds
You can cancel you plan mid-term if you: 
  • you cannot get an appointment within 30 days (due to the dental practice being unavailable, excludes period 15 Dec to 15 Jan) then we will refund your subscriptions for your most recent plan year less the cost for the services provided in your most recent plan year less a 3% processing fee
  • change your address - we will require proof of your new address and there are no refunds on subscriptions already paid
  • have not received any benefits in your most recent plan year however there are no refunds on subscriptions already paid unless you cancel within 30 days of signing up, in which case you will receive a refund less a 3% processing fee
We reserve the right to terminate your plan at any time and for any reason, including non-payment. If you have:
  • not received any benefits in your most recent plan year, we will refund your subscriptions for your most recent plan year
  • received any benefits in your most recent plan year, we will refund your subscriptions for your most recent plan year less the cost for the services provided in your most recent plan year less a 3% processing fee

Limitations, Exclusions & Exceptions
Haler Health is not a licensed insurer, health maintenance organization, or other underwriter of health care services. We are not licensed to provide and do not provide medical services or items to individuals.

Complaints
If there is an issue, problem or request for refunds, please contact Haler Health. If you have any complaints relating to professional services provided by the dental practice, please contact your relevant dental practice.
